Detroit Wayne Integrated Health Network and Community Living Services have partnered to develop a training overview for Clinically Responsible Service Providers in Wayne County on Person-Centered Planning (PCP) and Individual Plan of Service (IPOS). Please join us for this distance learning opportunity on the Zoom platform. 

Abstract: Navigating life transitions can feel overwhelming for everyone, including individuals with intellectual and developmental disabilities (IDD) and their families. This session is designed to equip the attendees with practical strategies and resources to make these changes smoother and more empowering. From moving from school to adult programs, transitioning from home to independent living, and understanding entitlements, this training will show you how person-centered planning can turn uncertainty into opportunity. Learn how to create plans that honor choice, build independence, and foster meaningful connections because every transition is a chance for growth.

By the end of this session, participants will be able to:
  1. Explain the principles of Person-Centered Planning (PCP) and how they support individuals with IDD during major life transitions.
  2. Identify key transition points such as school to adult services, home to independent living, and entitlement changes at age 18, and outline strategies to navigate them successfully.

  • TRAINING HOURS: (2) Event hours will count toward the annual training hours required by most provider agencies.
