Abstract: This is an overview of evidence-based wellness self-management strategies for community mental health settings.  The workshop will review research on the health impact of wellness self-management programs, including PATH (Planned Action Toward Health), WRAP(Wellness Recovery Action Plan), WHAM (Whole Health Action Management), INSIGHT and myStrength.  There will be actual practice presenting one tool-myStrength.

Objectives:

  1. To understand the role of health and wellness self-management programs for enrolled members of DWMHA who are at risk of chronic disease or chronic disease related complications or re-hospitalization. To explain how improved health literacy and clear health care communication enhances patient outcomes in treatment.
  2. To identify evidence-based or best practices chronic care improvement, chronic disease care, and whole person-centered planning in a community mental health setting.
  3. To define the term wellness self-management and be able to list at least three resource for obtaining self-management support tools and/or training
  4. Participants will have an opportunity to practice facilitating a portion of a Wellness Self-Management Training.
  5. To increase participants’ confidence in their ability to present a self-management wellness program for enrolled members to learn skills to manage their health and maintain active and fulfilling lives.
