Abstract: This is an overview of wellness self-management strategies for community mental health/substance use disorder treatment settings. The workshop will review best practices for integrating self-management into the individual plan of service/master treatment planning and ongoing treatment to improve outcomes utilizing a web-based wellness self-management tool - myStrength.

Objectives:

  1. To understand the role of health and wellness self-management programs for enrolled members of DWMHA who are at risk of chronic diseases, chronic disease related complications or re-hospitalization.
  2. To identify best practices for integrating wellness self-management into whole person-centered planning in a community mental health setting.
  3. To increase participants’ confidence in their ability to present a self-management wellness program for enrolled members to learn skills to manage their health and maintain active and fulfilling lives.
